To obtain injury report records from Recreational Sports, please complete the following:

Fully read, complete, and sign the Consent to Release Information Form
Email completed form to mdecember@ufl.edu (Michelle December Associate Director, Aquatics & Risk Management)

Please allow 3-5 business days for requests to be processed

If you have selected EMAIL as your method of correspondence, you will receive a confirmation email before any copies are sent.

If you have chosen FAX or US POSTAL SERVICE as your method of correspondence, paper copies of records will be sent to the designated address you provide on the form.
